Provider Practice Policies

APPOINTMENTS AND CANCELLATIONS
The standard meeting duration for initial/annual intakes is 45-60 minutes; duration for psychotherapy sessions is 45 minutes; duration for coaching sessions is 50-60 min. Please remember to cancel or reschedule a full 24 hours in advance. If you have a no show/missed appointment or late cancellation with less than 24 hours, you will be charged the “no show” fee and will be responsible for full payment of $60 fee, which will need to be paid prior to rescheduling another appointment.

Cancellations and last-minute rescheduled sessions will be subject to a full $60 “no show” charge if notice is not received AT LEAST A FULL 24 HOURS IN ADVANCE. This is necessary because a time commitment is made to you and is held exclusively for you. If you are late for a session, you will still be responsible for the full session rate regardless of any lost time for that session. The provider is not obligated to make up the missed time if you are late for a session or if there are technical difficulties with telehealth communication/technology.

Client agrees that if they repeatedly cancel or reschedule sessions, the provider has the sole discretion to terminate the therapeutic relationship. If the provider is the party needing to reschedule, the provider will make every effort to provide the client with at least 24 hours’ notice and will make every attempt possible to offer another appointment within 10 business days. Client acknowledges that the provider is not liable for any failure to perform or cancellations due to causes beyond its reasonable control including, but not limited to, acts of God, acts of civil authorities, acts of military authorities, riots, embargoes, acts of nature and natural disasters, endemics/pandemics, and other acts which may be due to unforeseen circumstances.
PAYMENT AND CREDIT CARD POLICY
The client acknowledges that payment for coaching or counseling services is due in full prior to or at the time of the appointment. Client agrees to maintain current and accurate credit card information on file for the purpose of payment of services as agreed upon, as well as for the charge of a $60 no show/late cancellation/missed appointment fee when applicable. Client agrees to ensure that the credit card information on file is current and up-to-date, and to immediately update credit card/payment information if/when the credit card expires, is cancelled, or payment is declined for any reason. Client acknowledges that an additional Declined/Late Payment Fee of $25 may be charged/added onto their account balance for any late payments or when a payment is declined for any reason. Client understands that services can be terminated at any time if they fail to make the required payments as designated in the purchased service package or per service as indicated. Client can cancel services and terminate the therapeutic relationship at any time, but client acknowledges that there will be no refunds for cancelled services that have been prepaid.
TELEPHONE ACCESSIBILITY
If you need to contact me between sessions, please leave a message on my voicemail or email me. I am often not immediately available; however, I will attempt to return your call within 24 hours. If a true emergency situation arises or you are at imminent risk of harming yourself or others, please call 911 or go to any local emergency room. You can also call 988 for the National Suicide and Crisis Lifeline.

ELECTRONIC COMMUNICATION
I cannot ensure the confidentiality of any form of communication through electronic media, including text messages, emails, and website platforms. If you prefer to communicate via email or text messaging for issues regarding scheduling or cancellations, I will do so. In the case of coaching services, there may be some additional out-of-session communication between client and coach via email or text messaging for the purposes of providing information, clarity, task reminders, encouragement/motivation, etc. as relevant to the client's coaching treatment and as discussed and agreed upon during sessions. For all electronic communications, while I may try to return messages in a timely manner, I cannot guarantee an immediate response and request that you do not use these methods of communication to discuss sensitive therapeutic content and/or request assistance for emergencies.
SOCIAL MEDIA AND TELECOMMUNICATION
Due to the importance of your confidentiality and the importance of minimizing dual relationships, I do not accept friend or contact requests from current or former clients on any social networking site (Facebook, LinkedIn, Snapchat, TikTok, Instagram, Twitter, etc.). I believe that adding clients as friends or contacts on these sites can compromise your confidentiality and our respective privacy. It may also blur the boundaries of our therapeutic relationship. If you have questions about this, please bring them up when we meet and we can talk more about it.

MINORS
If you are a minor, your parents/legal guardians may be legally entitled to some information about your therapy. I will discuss with you and your parents/legal guardians what information is appropriate for them to receive and which issues are more appropriately kept confidential.
TERMINATION
Ending therapeutic relationships can be difficult. Therefore, it is important to have a termination process in order to achieve some closure. I may terminate treatment after appropriate discussion with you if I determine that the psychotherapy or coaching services are not being effectively used or if you are in default on payment. I will not terminate the therapeutic relationship without first discussing and exploring the reasons and purpose of terminating. If therapy is terminated for any reason or you request another therapist, I will provide you with some outside referrals for qualified psychotherapists in your local area. You may also choose someone on your own or from another referral source.

Should you fail to schedule an appointment for three consecutive weeks, unless other arrangements have been made in advance, for legal and ethical reasons, I must consider the professional relationship discontinued.

Client acknowledges and agrees that noncompliance with the agreed upon policies in this contract or additional policies as outlined in other practice policies/forms may also result in immediate termination of services at the sole discretion of the provider. In all cases of termination, the client will be notified in writing of the effective date and reason for termination.
